Johan Anton Franck, Ph.D., ergothérapeute, chercheur

Le Professeur Franck est propriétaire du cabinet privé Franck-Handskills et ergothérapeute opérationnel à temps partiel au département de rééducation des lésions cérébrales du centre de rééducation Adelante (Pays-Bas).
Ses domaines d'intĂ©rĂȘt portent sur la gestion clinique de la rĂ©adaptation de l'Ă©paule, du bras et de la main dans les cas d'accidents vasculaires cĂ©rĂ©braux et de traumatismes crĂąniens, avec un intĂ©rĂȘt particulier pour les survivants d'accidents vasculaires cĂ©rĂ©braux souffrant d'une dĂ©ficience modĂ©rĂ©e Ă  sĂ©vĂšre du bras et de la main, avec des perspectives de rĂ©cupĂ©ration bras-main.

☛ Prof. Franck is an occupational therapist, senior researcher (PhD), owner of Franck-Handskills private practice and part-time operational OT at the Department of Brain Injury Rehabilitation of Adelante Rehabilitation Centre (Netherlands). 
His field of interest are: Clinical management of shoulder, arm and hand rehabilitation in stroke/TBI with a special interest in stroke survivors suffering from a moderately to severely impaired arm and hand with prospect regarding arm-hand recovery.

Le programme CARAS (Concise Arm and Hand Rehabilitation Approach in Stroke) est conçu pour permettre aux physiothĂ©rapeutes et aux ergothĂ©rapeutes de structurer et d’offrir un entraĂźnement du membre supĂ©rieur aux personnes ayant subi un AVC. Le programme CARAS s’appuie sur quatre concepts : 
a)    La stratification sur trois niveaux de sévérité des incapacités
b)    Les buts individuels de la personne et les résultats concomitants possibles de la réadaptation 
c)    Les principes d’auto-efficacitĂ© et 
d)    La capacitĂ© d’intĂ©grer de maniĂšre systĂ©matique et efficiente de (nouvelles) technologies et de nouveaux Ă©lĂ©ments d’entraĂźnement fondĂ©s sur des donnĂ©es probantes. 
Le cadre CARAS inclut trois programmes visant à traiter des déficits légers, modérés ou sévÚres du membre supérieur. Les thématiques sont; « Prendre soin du membre et prévenir les complications » (programme 1); « motricité globale et saisie orientée sur la tùche » (programme 2) ; et « entraßnement fonctionnel des habiletés du bras et des mains » (programme 3). 
La preuve de concept en rĂ©adaptation clinique a Ă©tĂ© faite. Le programme CARAS a Ă©tĂ© adoptĂ© par une majoritĂ© d’établissements de rĂ©adaptation aux Pays-Bas. Il permet de concevoir et de structurer rapidement des entraĂźnements du membre supĂ©rieur chez les personnes ayant subi un AVC. 

☛ The Concise Arm and hand Rehabilitation Approach in Stroke (CARAS) is designed for PTs & OTs to structure and implement arm-hand training in stroke survivors. CARAS is based on four constructs: 
a) stratification according to three levels of severity of arm– hand impairment, 
b) the individual’s rehabilitation goals and concomitant potential rehabilitation outcomes, 
c) principles of self-efficacy, and 
d) possibilities to systematically incorporate (new) technology and new evidence-based training elements swiftly. 

The framework encompasses three programs aimed at treating either the severely, moderately or mildly impaired arm-hand. Program themes are: ‘taking care of the limb and prevention of complications’ (program 1), ‘task-oriented gross motor grip performance’ (program 2) and ‘functional arm-hand-skill-performance training’ (program 3). 

Proof-of-principle in clinical rehabilitation has been established. CARAS is adopted by the majority of rehabilitation centers in the Netherlands and facilitates rapid structured design and provision of state-of-the-art arm-hand-function and arm-hand-skill-performance treatment in stroke patients.

L’objectif de la confĂ©rence d’aujourd’hui est de sensibiliser les cliniciens et chercheurs Ă  l’approche CARAS, d’échanger avec le Prof. Franck et de vĂ©rifier si cette approche prĂ©sente un intĂ©rĂȘt clinique et scientifique pour les milieux du CRIR. S’il y a un intĂ©rĂȘt partagĂ© pour un maillage de recherche clinique des dĂ©marches additionnelles pourraient ĂȘtre envisagĂ©es suite Ă  la rencontre. 
Cette confĂ©rence IURDPM-CRIR vise les intervenants intĂ©ressĂ©s par la rĂ©adaptation du membre supĂ©rieur, notamment aux programmes AVC et autres neuros des diffĂ©rents sites du CRIR.Â
